Job summary

Enhabit Home Health & Hospice seeks a qualified professional to represent the branch in external transitional care activities and to promote available hospice services. You will develop patient-centered transition plans by integrating clinical guidelines and metrics to ensure quality and efficiency.

The role requires licensing in the practicing state, at least one year in home care, and solid knowledge of federal, state, and local regulations governing hospice operations; strong technology

Qualifications

• Must be a graduate of an approved school of clinical education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience in home care.
• Must have demonstrated knowledge of federal, state, and local laws and regulatory guidelines governing hospice operations.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Represent the branch in external transitional care activities with patients, families, physicians, hospitals, facilities, and other health groups to promote Enhabit Home Health & Hospice services.
• Develop patient-centered transition plans by integrating clinical guidelines, protocols and metrics to promote quality and efficiency.
• Serve as a public awareness representative and be responsible for education related to provided services.
