Job summary

Enhabit Home Health & Hospice is seeking a Branch Liaison to advance patient-centered transitional care. You will collaborate with patients, families, physicians, hospitals, and caregivers to inform them about Enhabit’s services and coordinate seamless care transitions.

Ideal candidates will bring clinical education credentials, licensure in the state of practice, and at least one year of home care experience, with strong technology skills for digital documentation and communication.

Qualifications

• Must be a graduate of an approved school of clinical education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience in home care.
• Must understand federal, state, and local laws and regulatory guidelines governing hospice operations.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Represent the branch in external transitional care activities with patients, families, physicians, hospitals, facilities, senior living communities, professional associations, and similar health groups and institutions, to apprise them of the availability of Enhabit Home Health & Hospice services.
• Develop patient-centered transition plans that promote quality and efficiency by integrating clinical guidelines and metrics.
• Serve as a public awareness representative and educate about provided services.
