Job summary

Enhabit Home Health & Hospice is seeking a qualified professional to advance transitional care efforts at the local branch. You will engage with patients, families, physicians, hospitals and community partners to promote the availability of Enhabit services and ensure smooth handoffs.

Responsibilities include developing patient-centered plans by applying clinical guidelines and collaborating with care teams, while acting as a public advocate for the organization and educating stakeholders on

Qualifications

• Must be a graduate of an approved school of clinical education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience in home care.
• Familiarity with federal, state and local laws governing hospice operations.
• Basic technology skills, including operation of a mobile device.

Responsibilities

• Represent the branch in external transitional care activities and professional contacts to apprise them of Enhabit Home Health & Hospice services.
• Develop patient-centered transition plans by integrating clinical guidelines and metrics.
• Serve as a public awareness representative and educate about provided services.
