Job summary

Enhabit Home Health & Hospice is seeking a qualified clinician to support transitional care and coordination across patients, families, and healthcare partners. You will help implement guidelines and tools to ensure quality and efficient delivery of home health and hospice services.

The role requires bedside clinical experience, state licensure, and comfort with mobile technology to coordinate care plans and educate stakeholders in a supportive environment.

Qualifications

• Must be a graduate of an approved school of nursing, therapy, or social work.
• Must be licensed in the state where they currently practice.
• Must have at least two years clinical experience.
• Must have demonstrated understanding of federal, state, and local laws and regulatory guidelines for home health or hospice.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Represent the branch in transitional care activities and professional contacts to inform stakeholders about Enhabit Home Health & Hospice services.
• Integrate clinical guidelines, tools, and other metrics in patient-centered transition plans to promote quality and efficiency of care.
• Serve as a public awareness representative and manage education related to hospice services and payor sources.
