Job summary

Enhabit Home Health & Hospice is seeking a qualified clinician to support external transitional care activities and collaborate with patients, families, physicians, and health facilities. The role focuses on educating stakeholders about available home health and hospice services, while aligning with clinical guidelines and regulatory requirements.

Qualifications

• Must be a graduate of an approved school of clinical education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience in home care.
• Must understand federal, state, and local laws and regulatory guidelines governing the operations of a hospice.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Represent the branch in external transitional care activities, and in professional contacts with patients, families, physicians, hospitals, facilities, senior living communities, professional associations, and similar health groups and institutions, to apprise them of the availability of Enhabit Home Health & Hospice services.
• Integrate clinical guidelines, protocols, and other metrics to develop patient-centered transition plans that promote quality and efficiency.
• Serve as a public awareness representative and will be responsible foreducation related to provided services.
