Job summary

Affinity Hospice Management LLC is seeking a Hospice Chaplain to provide spiritual care to patients and families. The role includes assessment, care planning, direct spiritual support, and coordination with community chaplains.

The chaplain will participate in the interdisciplinary team, maintain documentation, and assist with education and outreach, including recruitment of volunteers. This position requires formal theological education and CPE experience.

Qualifications

• A bachelor's degree in religious studies or a related subject is required.
• Master's degree in theology, divinity, or pastoral studies is required.
• Certificate or coursework in pastoral education is preferred.
• Strong verbal and written communication; ability to document care.

Responsibilities

• Assesses spiritual needs of patients and families and coordinates timely services.
• Contributes to individualized care plans through comprehensive spiritual assessment.
• Provides direct spiritual care to patients and families.
• Serves as liaison to community chaplains and spiritual counselors.
• Maintains records of spiritual care services for QA and policy review.
• Documents communication with community chaplains and counselors.
• Attends interdisciplinary meetings and supports the care team.
• Provides education and consultation on spiritual care to the team.
• Recruits community chaplains and volunteers to meet patient needs.
• Plans funeral/memorial services as requested.
• Supports memorial services for staff and volunteers.
• Supervises spiritual care volunteers when assigned.
