Job summary

Affinity Hospice Management LLC seeks a Hospice Chaplain to deliver spiritual care to patients and families, coordinating with the hospice team to address identified spiritual needs. The role involves assessment, care planning, documentation, and collaboration with community chaplains.

The chaplain will work within an interdisciplinary group, participate in meetings, and support memorial services as requested, while maintaining professional education and community relations.

Qualifications

• Requires a bachelor's degree in religious studies and a master's in theology/divinity/pastoral studies.
• Preferred certification; ordination/endorsement by faith group often required.
• Commitment to ongoing education including CPE (Clinical Pastoral Education).
• Experience in hospice care is preferred.
• Ability to work in an interfaith setting and to respect diverse beliefs.

Responsibilities

• Provide direct spiritual care to patients and families/caregivers.
• Assess spiritual needs and develop plans of care accordingly.
• Coordinate with community chaplains and spiritual counselors.
• Document care interventions and communicate with IDG and physicians.
• Recruit and engage community clergy and spiritual care volunteers as needed.
• Plan memorial services and assist with funeral services when requested.
• Participate in quality improvement and interdisciplinary team activities.
