Job summary

Affinity Hospice Management LLC seeks a Hospice Chaplain to provide spiritual care for patients and families, coordinating with the hospice care team.

The role includes conducting spiritual assessments, developing care plans, and offering education and support to the interdisciplinary group. Recruits community chaplains and volunteers, arranges funeral services, attends IDG meetings, and participates in on-call duties as needed.

Qualifications

• Bachelor's degree in religious studies or related field required.
• Master's degree in theology, divinity or pastoral studies with 72 semester hours; CPE/Internship may be required.
• Certification is preferred; ordination/endorsement typically required for chaplaincy.

Responsibilities

• Provide direct spiritual care to patients and families/caregivers.
• Assess spiritual needs and develop care plans in coordination with the care team.
• Document care, participate in IDG meetings, and maintain records for QA.
• Recruit community chaplains and volunteers to meet patient needs; provide education through congregations.
• Plan and/or conduct funeral or memorial services per patient/family request; coordinate memorial services for staff and volunteers.
• Communicate with physicians, community counselors, and interdisciplinary group to ensure continuity of care.
• On-call duties and ongoing professional development; adhere to policy and regulatory requirements.
