Job summary

Affinity Hospice Management LLC seeks a Hospice Chaplain to provide spiritual care to patients and families, coordinating with other spiritual counselors as needed. The role includes direct care, community liaison, outreach, and memorial services.

The chaplain will collaborate with the interdisciplinary team, document care, and participate in quality assurance. Experience in hospice and interfaith settings is preferred, with strong communication and organizational skills.

Qualifications

• Bachelor's degree in religious studies and a master's in theology, divinity, or pastoral studies.
• Certification is preferred; ordained or commissioned chaplain with endorsement letter.
• Ongoing commitment to education and obtaining CPE; hospice care experience preferred.
• Ability to network with community clergy and congregations; comfort in an interfaith setting.

Responsibilities

• Assesses spiritual needs of patients and families and ensures timely services.
• Participates in comprehensive assessment to plan spiritual care.
• Provides direct spiritual care to patients and families/caregivers.
• Acts as liaison to community chaplains and spiritual counselors.
• Maintains records of spiritual care services for QA and program development.
• Documents services and communication with community chaplains.
• Attends IDG meetings and collaborates with the interdisciplinary group.
• Provides consultation, education, and support on spiritual care to the team.
• Recruits community chaplains and volunteers to meet needs.
• Provides for funeral or memorial services as requested; plans periodic memorial services.
• Supervises spiritual care volunteers when assigned.
