Job summary

Affinity Hospice Management LLC in Florida seeks a dedicated Hospice Chaplain to provide spiritual care for patients and families, directly or by coordinating with other chaplains and spiritual counselors.

The role includes spiritual assessments, care plan development, direct patient care, documentation, and collaboration with the interdisciplinary team. It also covers on-call duties, memorial services, and outreach to communities while fostering interfaith sensitivity and strong communication.

Qualifications

• Bachelor’s degree in religious studies or related subject and a Master’s degree in theology/divinity/pastoral studies required.
• Certification preferred; ordination/endorsement often required and four CPE units may be expected.
• Commitment to ongoing education and CPE.
• Hospice care experience preferred.
• Comfort working in an interfaith setting and respect for diverse beliefs.
• Ability to network with community clergy and congregations.
• Knowledge of hospice philosophy and care; strong communication and organizational skills.
• Offer contingent upon references and criminal background checks; physical ability to walk/stand.
• Must have a valid driver’s license and insured vehicle; CPR certification may be required.

Responsibilities

• Provide direct spiritual care to patients and families; assess spiritual needs and plan care.
• Coordinate with the interdisciplinary group to meet patient needs.
• Attend care conferences and document services rendered.
• Recruit community chaplains and volunteers to meet patient needs; provide education in communities.
• Conduct funeral or memorial services as requested; plan memorials for staff and volunteers.
• Supervise and support spiritual care volunteers as assigned.
