Affinity Hospice Management LLC is seeking a Hospice Chaplain to provide spiritual care to patients and families, coordinating with the interdisciplinary team. The role includes assessments, care planning, and liaison with community chaplains.

The chaplain will participate in care conferences, document services, recruit volunteers, and assist with memorial services while upholding hospice philosophy and interfaith sensitivity.

Qualifications

• Bachelor's degree in religious studies or a related subject plus a master's degree in theology, divinity, or pastoral studies with 72 semester hours and an internship or one unit of CPE.
• Certification is preferred, requiring ordination or commission to minister and ongoing education.

Responsibilities

• Conduct spiritual assessments of patients and families.
• Develop individualized plans of care addressing spiritual needs.
• Provide direct spiritual care to patients and families.
• Serve as liaison to community chaplains and spiritual counselors.
• Maintain records of spiritual care services and related activities for QA and policy review.
• Document direct services and communication with community chaplains.
• Attend patient care conferences and interdisciplinary group meetings.
• Provide consultation and education on spiritual care to the interdisciplinary group.
• Recruit community chaplains and spiritual care volunteers to meet needs.
• Arrange funeral or memorial services and periodic memorials as needed.
• Assist in supervising spiritual care volunteers.
