Job summary

Qualifications

• Master’s degree in Social Work with experience in healthcare.
• Ability to connect with patients, families, and caregivers during life’s most sensitive moments.
• Strong communication skills to liaise with families, medical teams, and community resources.
• Able to work independently while collaborating with a multidisciplinary team.
• Sensitivity to diverse cultural needs of patients and families.

Responsibilities

• Provide expert social evaluations, identifying psychosocial challenges such as caregiver stress and family dynamics.
• Offer compassionate counseling, supporting patients and families through emotional difficulties and crises.
• Collaborate with the care team to develop personalized care plans and share psychosocial insights.
• Facilitate community connections by linking families with local resources and ensuring seamless home care services.
• Support grieving families by coordinating a bereavement program for up to one year after a loved one’s passing.
• Drive operational excellence by leading daily social services operations and ensuring regulatory compliance.
