Qualifications

• MSW from CSWE-accredited program with healthcare experience.
• State licensure as required by practice location.

Responsibilities

• Explain hospice services and Medicare benefits; obtain informed consent.
• Submit documentation to facilities per policy and on time.
• Participate in the IDT; develop and implement the plan of care.
• Provide psycho-social support and counseling; refer to external resources as needed.
• Coordinate with IDT to ensure continuity and quality of care.
• Maintain up-to-date care plans reflecting problems, goals, and interventions.
• Assist in long-term care placement and monitor patient adjustment.
• Support advance planning including living will/POA documents.
