Job summary

Heritage Home Health & Hospice in Idaho Falls seeks a Licensed Social Worker to join our patient-centered care team. You will perform home visits, coordinate care with families and clinicians, and ensure compliance with agency policies to support patients at home.

The role emphasizes teamwork, ongoing education, and compassionate support for individuals and families navigating illness, aging, or end-of-life care.

Qualifications

• Licensed in Idaho as an LSW/LMSW/LCSW (or eligible).
• BLS CPR certified.
• Home Health or Hospice experience preferred, but not required.

Responsibilities

• Perform visits to patients in their place of residence and coordinate care.
• Follow agency, state, and federal policies and procedures.
• Collaborate with interdisciplinary team and community agencies to coordinate care.
