Job summary

Idaho Home Health and Hospice seeks a Social Worker - Hospice Per Diem to provide medical social services under physician direction. You will support patients and families, contribute to a comprehensive Plan of Care, and coordinate bereavement services as needed.

The role offers per diem hours with duties in coordination with an interdisciplinary team, and requires an MSW, healthcare experience, and a valid driver’s license. Benefits and per diem compensation are provided.

Qualifications

• Master's Degree in Social Work from a CSWE-accredited program.
• 1+ years of social work experience in a health care setting.
• Current driver's license, vehicle insurance, and access to a dependable vehicle.

Responsibilities

• Assist patients, families and care teams in understanding social and emotional factors affecting health.
• Contribute to an integrated Plan of Care on a daily basis.
• Instruct care team on community resources to support patients as needed.
• Function as Bereavement Coordinator and plan bereavement services if needed.
