Job summary

Deaconess Homecare (a part of LHC Group) is seeking a Social Worker - Hospice Per Diem to provide medical social services under physician direction and support the interdisciplinary team in addressing social and emotional factors affecting patient health and coping strategies.

The role offers an hourly pay range and a comprehensive benefits package; a full-time equivalent is considered with per diem scheduling. Candidates with CSL or licensure in the state are preferred.

Qualifications

• Master's Degree from a school of Social Work accredited by the Council on Social Work
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
• Licensed Social Worker in the state of residence (preferred)
• CPR certification (or ability to complete within 90 days) (preferred)

Responsibilities

• Provide medical social services under the direction of a physician and Interdisciplinary Group
• Assist in understanding significant social and emotional factors related to patient health status and development of coping mechanisms
