Job summary

UnitedHealth Group's Community Loving Care Hospice (an LHC Group/Optum family member) is seeking a Social Worker - Hospice Per Diem to deliver medical social services under physician direction. The role supports understanding social factors and aids in developing coping strategies for patients and families.

Responsibilities include collaborating with the care team to craft Plans of Care, educating on community resources, and potentially coordinating bereavement services as needed.

Qualifications

• Master's Degree from a CSWE-accredited school.
• 1+ years of social work experience in a health care setting.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.

Responsibilities

• Assist patient, families, physician and care team to understand significant personal, emotional, environmental and social factors related to health status.
• Develop a comprehensive, integrated Plan of Care for patients on a daily basis.
• Instruct care team on community resources available to assist patients.
• Serve as Bereavement Coordinator and establish a care plan addressing bereavement needs.
