Job summary

Community Loving Care Hospice, part of the LHC Group, seeks a Social Worker - Hospice Per Diem to provide medical social services under physician direction and an Interdisciplinary Group to assist in understanding social and emotional factors related to the patient's health and coping needs.

The role emphasizes planning of care, bereavement coordination when needed, and collaboration with the health care team to support patients and families.

Qualifications

• Master's Degree in Social Work from an accredited school.
• 1+ years of social work experience in a health care setting.
• Current driver's license, vehicle insurance, and access to reliable transportation.

Responsibilities

• Assists patients and families to understand social and emotional factors affecting health.
• Contributes to a comprehensive, integrated Plan of Care daily.
• Instructs the care team on available community resources as needed.
• Function as Bereavement Coordinator and address bereavement needs with a care plan.
