UnitedHealth Group's Idaho Home Health and Hospice is seeking a Social Worker - Hospice Per Diem to provide medical social services under physician oversight and a collaborative care team, addressing emotional and social factors affecting health and coping.

The role emphasizes developing and implementing a Plan of Care, coordinating bereavement services as needed, and guiding the team with community resources. A Master’s in Social Work and licensure are preferred.

Qualifications

• Master's Degree from a school of Social Work accredited by the Council of Social Work
• 1+ years of social work experience in a health care setting
• Current Driver's License, vehicle insurance, and access to dependable transportation

Responsibilities

• Assist patients and families in understanding social and emotional factors related to health status
• Contribute to the development of a comprehensive Plan of Care daily
• Instruct care team on community resources available to assist patients
