• Home Health and Hospice experience preferred.

• Assesses psychosocial status of patients and families/caregivers related to health problems.
• Carries out social evaluations and plans intervention based on evaluation findings.
• Maintains clinical records on all patients referred to social work.
• Provides information and referral services for patients and families.
• Acts as liaison between patients, families, and community agencies.
• Maintains collaborative relationships with organization personnel to support patient care.
• Maintains and develops contracts with public and private agencies as resources.
• Assists volunteer coordinator in establishing need for volunteers.
• Participates in development of individualized plan of care and team meetings.
• Adheres to documentation and care procedures and standards.
• Assists physicians and team in understanding social and emotional factors related to health problems.

Ausbildung

LMSW/LSW licensed in Idaho

Oregon licensure within reasonable time

Valid Driver's License

Home Health and Hospice experience preferred
