Job summary

ACG Hospice is seeking a licensed Medical Social Worker to perform psychosocial evaluations and provide ongoing counseling to patients and families. You will support them through the end-of-life journey and work with the care team to implement the plan of care.

Requirements include an MSW from an accredited program, current state licensure, and 2+ years of clinical experience in healthcare or hospice. Reliable transportation is essential.

Qualifications

• MSW from an accredited school of social work approved by CSWE
• Current state social work license
• 2+ years of clinical experience in healthcare or hospice
• Reliable transportation and ability to lift up to 25 lbs

Responsibilities

• Perform psychosocial evaluations for patients and families
• Provide ongoing counseling and emotional support as part of the plan of care
• Support patients and families through end-of-life journey
• Collaborate with care team to coordinate resources and services
