Job summary

ACG Hospice is seeking a dedicated Social Worker to provide psychosocial assessments and ongoing counseling to patients and families facing end-of-life care. The role emphasizes compassionate support aligned with the plan of care and teamwork within hospice services.

Qualified candidates hold an MS in social work, a current state license, and at least 2 years of clinical experience. Travel requirements and physical ability to assist patients are expected.

Qualifications

• Heart to serve patients and families; compassion and empathy in care delivery.
• Education: MS degree in social work from an accredited school of social work approved by the CSWE.
• Licensure: Current state license as a social worker.
• Experience: 2+ years of clinical work experience, preferably in healthcare or hospice.
• Required: Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 25lbs.

Responsibilities

• Conduct psychosocial evaluations and provide ongoing counseling to patients and families during end-of-life care.
• Support patients and families in navigating the plan of care and accessing bereavement resources as needed.
• Collaborate with interdisciplinary team to deliver compassionate hospice care and ensure quality patient experiences.
