Job summary

Agape Care Group is seeking a Hospice Social Worker to provide psychosocial evaluations and ongoing counseling for patients and families facing end-of-life care. You will support emotional well-being, collaborate with the care team, and implement the plan of care with compassion.

The role requires an MSW from an accredited program, an active state social work license, and at least 2 years of clinical hospice experience, along with reliable transportation and a passion for empathetic care.

Qualifications

• A heart to serve patients and families and a passion for providing the best possible care
• Experience: 2+ years of clinical work experience, preferably in healthcare or hospice
• Licensure: Current state license as a social worker
• Education: MS degree in social work from an accredited school of social work approved by the CSWE
• Required: Reliable transportation
