Job summary

Inspire Hospice & Palliative Care, part of Cadre Family Companies, seeks a Hospice Social Worker (MSW) to provide psychosocial support to patients and families navigating end-of-life care. Travel across the assigned service area is required, with potential on-call duties.

MSW degree with relevant experience is essential; preferred candidates may hold LCSW/LMSW. You will join a collaborative interdisciplinary team dedicated to dignified, person-centered care for those facing advanced illness.

Qualifications

• Master's degree in Social Work (MSW) from an accredited program.
• Minimum of one (1) year of experience in social work, counseling, or a related healthcare field.
• Current automobile insurance and a valid driver's license.
• Reliable transportation and the ability to travel throughout the assigned service area.
• Strong understanding of grief, loss, psychosocial dynamics, and family systems.
• Excellent communication, interpersonal, and counseling skills.
• Ability to establish rapport with patients and families facing end-of-life issues.

Responsibilities

• Conduct comprehensive psychosocial assessments of hospice patients and families within required timeframes.
• Assess coping mechanisms, support systems, strengths, preferences, and resources.
• Develop individualized care plans addressing identified psychosocial needs.
• Provide compassionate, culturally sensitive support throughout the hospice journey.
• Provide individual and family counseling related to grief, loss, and end-of-life decisions.
• Coordinate with the interdisciplinary team to support continuity of care.
