LiveWell Partners seeks a Hospice Medical Social Worker (MSW) to provide compassionate psychosocial support to patients and families across hospice care. The MSW will assess emotional, social, financial, and environmental needs, developing individualized care plans with the interdisciplinary team.

Responsibilities include comprehensive psychosocial assessments, counseling, end-of-life planning, bereavement support, and community resource linkage.

Qualifications

• Licensed medical social worker in Michigan required.
• MSW degree required.
• Minimum 1 year of licensed MSW experience in healthcare setting required.
• 2+ years in acute or post-acute care preferred.

Responsibilities

• Conduct comprehensive psychosocial assessments of hospice patients and families.
• Develop and implement individualized social work interventions and counseling.
• Educate patients and families about hospice services, end-of-life planning, and resources.
• Connect patients with community resources and advocate for patients and families.
• Collaborate with interdisciplinary team to address psychosocial needs and family dynamics.
• Provide bereavement support and referrals to resources.
