Qualifications

• MSW from a school accredited by the CSWE.
• Minimum of 1 year experience in health care; hospice experience preferred.
• Strong verbal and written communication and organization skills.
• References and criminal background checks required; driving license with insured vehicle.

Responsibilities

• Assess psychosocial status of patients and families; communicate findings to the team.
• Perform social evaluations; plan interventions and crisis support as needed.
• Educate on advanced directives; participate in care planning with patient/family.
• Assist in planning for funeral, financial, legal, and health decisions.
• Maintain accurate clinical notes; coordinate with physicians and care team.
• Attend interdisciplinary group meetings and ensure continuity of care.
• Act as liaison with community agencies and maintain required reports.
