Job summary

Affinity Hospice Management LLC is seeking a Medical Social Worker (MSW) to implement standards of care for hospice services. The MSW will join the interdisciplinary team to provide psychosocial support based on ongoing assessments and goals within the plan of care.

The role requires an MSW, OH licensure, and strong verbal/written communication. Hospice experience is preferred; CPR certification is expected and COVID-19 vaccination is required where applicable.

Qualifications

• MSW from a CSWE-accredited program.
• Must be licensed to practice social work in OH.
• Minimum 1 year health care or hospice experience preferred.
• Strong verbal and written communication skills.
• CPR certification required.
• COVID-19 vaccination preferred/required per policy.

Responsibilities

• Assess psychosocial status of patients and families and communicate findings to the Interdisciplinary Group.
• Conduct social evaluations including family dynamics and caregiver abilities; plan interventions.
• Educate on culture-specific needs and advanced directives.
• Contribute to development of the plan of care and participate in group meetings.
• Assist with funeral planning, finances, legal, and healthcare decision responsibilities.
• Provide crisis intervention and maintain a safe environment for patients.
