Compassus is seeking a Hospice Social Worker in Texas to provide psychosocial support, education, and evaluation for patients and families. You will collaborate with the interdisciplinary team to develop and implement comprehensive care plans, assist with housing, insurance, and end-of-life planning, and ensure documentation meets policy timelines.

The role requires an MSW from a CSWE-accredited program with at least 1 year of healthcare experience or a BA in social work with supervision

Competenze

MSW from a CSWE-accredited program with at least 1 year of healthcare experience.
Bachelor’s degree in social work or related field with 1 year of healthcare experience eligible for supervision.
State licensure as required for the practicing state.

Mansioni

Explain hospice services and Medicare benefits to patients and families; obtain consent and election documents.
Submit documentation to facilities after patient visits per policy.
Participate in the IDT and develop/execute the plan of care.
Provide psycho-social support and counseling to patients and families; refer to external resources as needed.
Identify interventions needed by other IDT members and coordinate care.
Communicate patient and family needs to IDT; coordinate psycho-social care and resources.
Maintain up-to-date care plans reflecting problems, goals, and interventions.
Facilitate long-term care placement when warranted and monitor adjustment.
Assist with advance planning incl. living will/POA, funeral arrangements, and service planning.
Perform other duties as assigned.
