Job summary

Compassus is seeking a Hospice Social Worker to support patients and families by providing psychosocial assessments, counseling, and care coordination within the interdisciplinary team.

The role requires an MSW from a CSWE-accredited program (or BSW with MSW supervision availability), with healthcare experience and state licensure as applicable. Candidates will collaborate across departments to ensure compassionate, comprehensive care for each patient.

Qualifications

• MSW from a CSWE-accredited program with at least 1 year of healthcare experience.
• Candidates with a bachelor’s degree in social work and 1 year of healthcare experience will be considered where MSW supervision is available.
• Must hold state licensure as required for the state in which you practice.

Responsibilities

• Explains hospice services and Medicare benefits to patients and families; obtains consent and election documents.
• Submits documentation to facilities per policy and timelines.
• Participates as a member of the IDT and contributes to the plan of care.
• Provides psycho-social support and counseling to patient and family; coordinates external resources as needed.
• Maintains comprehensive care plans reflecting problems, goals, and interventions.
• Facilitates advance planning including living will/POA, funeral arrangements, and body/organ donation.
