Job summary

Compassus is seeking a Hospice Social Worker to provide psychosocial support and counseling to patients and families. You will collaborate with the interdisciplinary team to develop and implement care plans, coordinate resources, and promote the Compassus philosophy of compassionate care across the continuum.

The role requires an MSW (CSWE-accredited) with healthcare experience and applicable state licensure.

Qualifications

• MSW from a CSWE accredited program with at least 1 year healthcare experience.
• Candidates with a bachelor’s degree in social work and 1 year healthcare experience may be considered where MSW supervision is available.
• State licensure as required for the practice state.

Responsibilities

• Explain hospice services and Medicare benefits to patients and families; obtain consent and election documents.
• Submit documentation timely according to policy.
• Participate as IDT member in plan of care development and implementation.
• Provide psycho-social support and counseling to patients and families; coordinate external resources as needed.
• Maintain comprehensive, updated care plans reflecting problems, goals and interventions.
• Facilitate advance planning including living wills and POA documents.
