Job summary

Compassus is seeking a Hospice Social Worker to deliver psychosocial assessment, counseling, and family support. You will collaborate with the IDT to plan and implement care and help families understand Medicare benefits and placement options.

The role requires an MSW from a CSWE-accredited program and state licensure where applicable, with healthcare experience and strong communication skills. This is an on-site position focused on compassionate, inclusive care.

Qualifications

• MSW from CSWE-accredited program with healthcare experience
• State licensure as required (e.g., TX)
• Clear ability to read, write, and communicate effectively with patients and families

Responsibilities

• Explain hospice services and Medicare benefits to patients and families
• Submit documentation timely after visits
• Participate in IDT care planning and implementation
• Provide psychosocial support and counseling to patients and families
• Coordinate with IDT for continuity and quality of care
• Maintain updated care plans reflecting problems, goals, and interventions
• Assist in long‑term care placement and advance planning
