Job summary

Compassus is seeking a Hospice Social Worker to address the psychosocial needs of patients and families. You will collaborate with the interdisciplinary team to plan and implement care, provide counseling, and coordinate community resources.

The role emphasizes compassionate, non-discriminatory support across diverse situations and care settings. Candidates must hold an MSW and state licensure, with healthcare experience preferred.

Qualifications

• MSW from CSWE-accredited program with healthcare experience preferred.
• Licensure as required by the practicing state is mandatory.

Responsibilities

• Explain hospice services and Medicare benefits to patients and families; obtain informed consent and elect benefits documents.
• Submit documentation to facilities upon patient visits; ensure timely completion per policy.
• Participate as a member of the IDT and contribute to the care plan.
• Provide psycho-social support and counseling; coordinate with external resources as needed.
• Identify needs for intervention by other IDT members and communicate patient/family needs.
• Collaborate with IDT to ensure continuity and quality of psycho-social care.
