Job summary

Compassus is seeking a Hospice Social Worker in the Reston area to provide psychosocial care to patients and families. You will collaborate with the IDT, coordinate planning, and support end-of-life care consistent with Compassus values.

The role requires an MSW with healthcare experience and state licensure; you will assess needs, link to community resources, and participate in care planning and advance directives.

Qualifications

• MSW from a CSWE-accredited program with at least 1 year of healthcare experience.
• Bachelor's degree in social work or related field with 1 year healthcare experience considered where MSW supervision is available.
• Must hold state licensure as required for the state in which you practice.

Responsibilities

• Explain hospice services and Medicare benefits; obtain informed consent and election documents as requested.
• Submit appropriate documentation to facilities upon patient visits; ensure timely submission according to policy.
• Participate as a member of the IDT and contribute to the care plan implementation.
• Provide psycho-social support and counseling to patient and family; coordinate external counseling resources as needed.
• Identify the need for intervention by other IDT members.
• Effectively communicate patient and family needs to IDT.
• Coordinate psycho-social care with IDT to ensure continuity and quality of care.
• Maintain comprehensive care plans reflecting current problems, goals, and interventions.
• Respect family environments and beliefs; practice nondiscrimination.
• Complete initial and ongoing psycho-social assessments; address financial, living arrangements, caregiver stress, pre-bereavement, and community resource needs.
• Facilitate placement in long-term care as warranted and monitor adjustment.
• Participate in care planning meetings at long-term care facilities.
• Support advance planning including living wills/POA, funeral arrangements, and body/organ donation.
• Performs other duties as assigned.
