Compassus is seeking a dedicated Hospice Social Worker to model our values and support patients and families with psychosocial care. You will collaborate with the interdisciplinary team to address emotional, social, and financial needs within a hospice setting.

The role emphasizes comprehensive assessments, care planning, placement assistance, and respectful, nondiscriminatory service across facilities, with licensure requirements aligned to state regulations.

Qualifikationen

• MSW from a CSWE-accredited program with at least 1 year of healthcare experience.
• State licensure as required and applicable for the state in which you practice.
• Strong communication and counseling skills.

Aufgaben

• Explains hospice services and Medicare benefits; obtains informed consent and election of benefits documents.
• Submits documentation to facilities per policy and ensures timely completion.
• Participates as a member of the IDT and contributes to the plan of care.
• Provides psycho-social support and counseling to patient and family; facilitates external resources as needed.
• Assesses psycho-social needs, including financial, housing, caregiver stress, and community resources.
• Facilitates placement in long-term care when warranted and monitors adjustment.
• Participates in care planning meetings and advance planning activities.
