Job summary

Compassus is seeking a Hospice Social Worker to provide preventive, educational, evaluative and treatment services to meet the psychosocial needs of patients and families, helping them improve, restore, and maintain coping, in collaboration with the interdisciplinary team.

The role includes conducting psycho-social assessments, supporting advance planning, coordinating with long-term care and community resources, and maintaining care plans, while upholding nondiscrimination and compassionate

Qualifications

• Master of Social Work from a CSWE-accredited program with at least 1 year of healthcare experience.
• Candidates with a social work bachelor's degree and 1 year healthcare experience may be considered where MSW supervision is available.
• State licensure as required for the practice location.

Responsibilities

• Explain hospice services and Medicare benefits; obtain informed consent and election of benefits documents.
• Submit documentation to facilities after patient visits; ensure timely submissions per policy.
• Participate as an IDT member; develop and implement the plan of care.
• Provide psychosocial support and counseling; refer to external resources as needed.
• Identify need for intervention by other IDT members; communicate needs to the IDT.
• Coordinate psychosocial care with the IDT; maintain comprehensive care plans reflecting current problems and goals.
