Job summary

Carle Health is seeking a Home Services Social Worker to identify psychosocial needs of patients and families and coordinate support services. The role requires hospice experience of 1+ years and collaboration with interdisciplinary teams to deliver comprehensive care.

The position offers a sign-on bonus and requires a Master’s in Social Work with licensure as an LSW or LCSW preferred. A valid driver’s license is also required to operate within communities served.

Qualifications

• Master's Degree in Social Work required.
• Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) preferred.
• Driver's license and auto insurance as required.

Responsibilities

• Provide psychosocial assessments of patients and families to identify needs.
• Develop a bereavement plan of care for families and caregivers.
• Educate patients and families, addressing learning needs and barriers.
• Document assessments and interventions in the electronic medical record.
• Collaborate with interdisciplinary teams to support care plans.
