Job summary

Advocate Health is hiring a psychosocial care specialist for the Atrium Health Hospice Cabarrus - Patient and Family Services in Kannapolis, NC. The role conducts psychosocial evaluations, provides counseling, and collaborates with a multidisciplinary team to plan patient care across the continuum. Requires a Master’s in Social Work; an LCSW is preferred.

Hospital or health system experience is preferred. The position is full-time, 40 hours/week, with 8:30-5 schedule and on-call requirements;

Qualifications

• Master’s degree in Social Work required.
• LCSW preferred.
• At least 1 year hospital/health setting experience is preferred.

Responsibilities

• Interviews patients and family to complete psychosocial evaluations.
• Provides counseling and education to patients and families.
• Collaborates with multidisciplinary team to plan care across the continuum.
• Serves as liaison to hospital staff on psychosocial aspects of illness.
• Reports abuse/neglect; coordinates with Department of Social Services.
• Identifies barriers to transition and facilitates system improvements.
