Job summary

aah is seeking a licensed clinical social worker to join the GCMG Hospice and Palliative: Cabarrus team. The role focuses on biopsychosocial assessments, care planning, and counseling for patients and families within a fast-paced hospice setting.

You will provide psychosocial evaluations, coordinate with multidisciplinary staff, and mentor junior social workers while advocating for patients throughout the care continuum. A CSWE-accredited MSW and state CSW license are required.

Qualifications

• Master's Degree in Social Work from a CSWE approved program.
• License in Clinical Social Work required in applicable state/SC.

Responsibilities

• Provides biopsychosocial assessments, diagnoses, treatment planning and referrals.
• Interviews patient and social support to complete psychosocial evaluation.
• Provides brief psychotherapy, supportive counseling and education to patient/family.
• Advocates for patient and family throughout care continuum.
• Mentors new social workers and provides clinical supervision as needed.
