Job summary

Atrium Health Wake Forest Baptist seeks a qualified social worker to provide comprehensive psychosocial assessments and evidence-based interventions in a full-time palliative care setting. You will conduct biopsychosocial evaluations, plan and monitor patient- and family-centered care, and collaborate with a multidisciplinary team.

Responsibilities include brief psychotherapy, advocacy across the care continuum, and supervision of junior staff, with documentation aligned to departmental

Qualifications

• Master's Degree in Social Work from a CSWE-accredited program.
• Licensed Clinical Social Worker in applicable state; SC license may be required.
• Ability to mentor/supervise new social workers as needed.

Responsibilities

• Provides biopsychosocial assessments, diagnoses, treatment planning, and referrals.
• Interviews patients and support systems for psychosocial evaluations.
• Provides brief psychotherapy, supportive counseling, and education to patients and families.
• Coordinate aspects of care for patient/family with the multidisciplinary team.
• Develops and revises care plans based on ongoing assessments.
• Educates and advocates for patients and families across the care continuum.
• Document accurately and timely per policy.
• Provide supervision to provisionally licensed social workers.
• Participate in clinical pathways, protocols, and population-based programs.
• Collect and interpret data from records to assess outcomes.
