Job summary

VIA Health Partners in Charlotte, NC, is seeking a qualified social worker to provide psychosocial assessments and comprehensive care planning for patients and families at end of life. The role emphasizes advocacy, resource coordination, and culturally sensitive care within an interdisciplinary team.

Minimum qualifications include a bachelor's in social work or a master’s in social work (CSWE-accredited), with licensure as LCSWA/LCSW in NC and LISW-CP in SC as applicable.

Responsibilities

• Conducts psychosocial assessments of patients/caregivers and families to identify emotional, social, and environmental strengths and problems related to diagnosis and life situation.
• Develops, implements, and evaluates plans of care for patients/caregivers and families, addressing counseling and education needs while maintaining the dignity of the dying patient.
• Addresses patient and caregiver needs for resource and referral services and advocacy; updates plan of care as goals are achieved or changed.
• Collaborates with interdisciplinary teams to provide culturally sensitive care and to coordinate services.
• Keeps clinical records current by completing and synchronizing electronic medical records throughout the day.
• Attends meetings, participates in process improvements, and maintains open communication with Supervisor.
• Maintains NC LCSWA/LCSW licensure (and SC LISW-CP where applicable) and pursues professional development.
