Job summary

Advocate Health seeks a dedicated social worker to perform psychosocial assessments, counseling, and crisis intervention within the Patient and Family Services department in Kannapolis, NC. The role requires a Master’s in Social Work and likely an LCSW; hospital experience is preferred.

Clinicians will work with a multidisciplinary team to plan patient care, coordinate community resources, and ensure smooth transitions across the healthcare continuum.

Qualifications

• Master's degree in Social Work; LCSW preferred.
• 1+ year hospital/health setting experience preferred.
• Experience with data management tools.
• SC licensure requirements may apply if working in SC.

Responsibilities

• Interview patient and family to complete psychosocial evaluation.
• Provide counseling and support to patient and family.
• Collaborate with the care team to implement a psychosocial care plan.
• Serve as liaison to social services during evaluation/investigation.
• Identify barriers to care and assist with system improvements.
• Maintain accurate case records and documentation per standards.
