Responsibilities

• Interviews patient and social support system to complete psychosocial evaluation.
• Provides counseling and education to patients and families to cope with hospitalization, disability or illness.
• Collaborates with multidisciplinary team to implement care plans addressing psychosocial needs.
• Serves as resource to staff and physicians on emotional and social aspects of illness.
• Intervenes in crisis situations using appropriate crisis intervention skills.
• Completes psychosocial assessments to guide post-hospital care for patients and families.
• Assesses level of care needs with Clinical Care Management for appropriate setting across continuum.
• Participates in multidisciplinary care planning meetings.
• Researches community resources for social, emotional, or financial assistance.
• Educates patients and families on community options and makes referrals as needed.
• Reports suspected abuse/neglect and liaises with Social Services during evaluation.
• Identifies barriers to transitions across the healthcare continuum and supports improvements.
• Maintains accurate case records and documents in medical records per standards.
