Job summary

Atrium Health is seeking a social worker with a Master’s degree in Social Work to conduct psychosocial evaluations and provide counseling for patients and their families in a hospital setting in Kannapolis, NC. The role collaborates with the Clinical Care Management team and multidisciplinary staff to plan and implement care addressing psychosocial needs.

The position requires crisis intervention capabilities, knowledge of community resources, and adherence to NASW ethics.

Qualifications

• Master's degree in Social Work required or preferred.
• LCSW licensure preferred or applicable state license.
• Experience in hospital or health-care setting preferred.

Responsibilities

• Interviews patient and social support to complete psychosocial evaluation.
• Provides counseling and education to patient and social support to cope with hospitalization and illness.
• Collaborates with multidisciplinary team to plan care addressing psychosocial needs.
• Serves as a resource on emotional and social aspects of illness for staff and physicians.
• Intervenes in crisis situations using crisis intervention techniques.
• Completes psychosocial assessments for post-hospital care.
• Assesses appropriate level of care with Clinical Care Management for continuum of care.
• Participates in multidisciplinary care planning meetings.
• Maintains knowledge of community resources for social, emotional, or financial assistance.
• Educates patients on community resources and assists with referrals to community agencies as needed.
• Reports suspected abuse/neglect/exploitation and liaises with Department of Social Services.
• Identifies barriers to smooth transition across healthcare continuum and suggests improvements.
• Maintains accurate case records per departmental standards.
