Job summary

Advocate Health Hospice Cabarrus – Patient and Family Services in Kannapolis, NC, seeks a full-time Social Worker with a Master’s in Social Work (LCSW preferred). Duties include psychosocial assessments, counseling patients and families, crisis intervention, and coordinating with a multidisciplinary team for post-discharge planning.

Hours are 40 per week, Monday–Friday, 8:30–5 with on-call responsibilities.

Qualifications

• Master's degree in Social Work required; LCSW preferred.
• Interviews patient and family; psychosocial evaluation.
• Provides counseling and education to patient and family.
• Works with multidisciplinary team to plan care.

Responsibilities

• Interviews patient/patient's social support to complete psychosocial evaluation.
• Provides counseling and education to patient and family.
• Collaborates with the team to implement a care plan addressing psychosocial needs.
• Acts as liaison with hospital staff and Department of Social Services as needed.
• Maintains case records according to department standards.
