Job summary

The role collaborates with the Clinical Care Management team to determine appropriate care level, provides education on available resources, and ensures protection reporting; requires a Master’s in Social Work with LCSW preferred and hospital experience.

Qualifications

• Master of Social Work required for hospital social work.
• LCSW certification preferred when available.
• Experience in hospital or health-related setting is preferred.

Responsibilities

• Complete psychosocial assessments for post-hospital care.
• Assist patients with coping and adjustment to illness and hospitalization.
• Collaborate with Clinical Care Management to determine level of care.
• Educate patients on community resources and make referrals.
• Report suspected abuse/neglect/exploitation and liaise with DSS.
• Respond to after-hours consult requests.
