Job summary

Advocate Health is seeking a Part-time Case Manager in Cabarrus County, NC, to support patients transitioning from hospital to community care. The role requires a Master’s in Social Work with an LCSW preferred, and involves collaboration with a clinical care team to assess patient needs and connect to services.

The position works Saturdays and Sundays with hours 7am–7:30pm and includes benefits eligible and holiday weekends coverage; compensation aligns with a pay range of $30.70–$46.05 per hour

Qualifications

• Master's in Social Work is required; LCSW preferred.
• Applicable state certification preferred.
• Adherence to NASW Code of Ethics.

Responsibilities

• Completes psychosocial assessments for post hospital care for designated patients and their support system.
• Assists patients in coping with stress related to hospitalization, disability, or chronic illness.
• Collaborates with Clinical Care Management to determine level of care or environment setting.
• Serves as a resource to staff and physicians on psychosocial aspects of illness and social support.
• Educates patients on community resources and post-hospital care options; makes referrals as needed.
• Reports suspected abuse/neglect/exploitation; liaises with DHS during investigations.
• Responds to after-hours consult requests.
