Job summary

Advocate Health Hospice Cabarrus is seeking a full-time Spiritual Care Provider to deliver compassionate spiritual support to patients and families in Kannapolis and surrounding areas. You will assess spiritual needs, provide counseling and sacraments as appropriate, and collaborate with the interdisciplinary team to implement a personalized plan of care.

Qualifications

• Master's Degree in Theology, Divinity, or Religious Studies.
• ACPE; 2 CPE units required, 5 units preferred.
• Endorsement by appropriate religious authority.

Responsibilities

• Provides initial assessment of patient’s spiritual resources and preferences.
• Provides spiritual care and emotional support to patients and families.
• Offers pastoral counseling, sacraments, and support aligned with patient’s faith.
• Develops a spiritual care plan reflecting problems, interventions, and outcomes.
• Implements the plan with the hospice team following protocols and measures.
• Acts as liaison with the patient’s faith community.
