Job summary

Carle Health is seeking a Home Services Social Worker to assess and address the psychosocial needs of patients and families. The role collaborates with interdisciplinary teams and requires hospice experience of 1+ year. A Master’s in Social Work and LSW/LCSW licensure are expected, with a valid driver’s license.

Qualifications

Master's Degree in Social Work required.
Licensed Social Worker (LSW) and Licensed Clinical Social Worker (LCSW) licensure (IDFPR) preferred.
Hospice experience of 1+ year required.
Driver's License and auto insurance required.

Responsibilities

Provide psychosocial assessments of patients and families to identify emotional, social, and environmental needs related to diagnosis, illness, and life situation.
Develop a bereavement plan of care addressing family/caregiver needs.
Educate patients and families, addressing barriers and matching learning levels.
Document assessments and MSW interventions in the patient’s EMR.
Implement and adjust social work plan to maximize benefits and coping for patients and families.
