Job summary

Carle Health is seeking a Home Services Social Worker to assess psychosocial needs of patients and families, develop care plans, and collaborate with interdisciplinary teams. This role emphasizes hospice experience and supportive counseling within a comprehensive palliative approach.

The successful candidate will hold a Master's in Social Work, be licensed or eligible as an LSW/LCSW, and demonstrate teamwork in a fast-paced environment. Sign-on bonus is offered and travel may be required.

Qualifications

• Master's Degree in Social Work or related field.
• Experience working in hospice or palliative care preferred.
• Ability to work as part of a multidisciplinary team.

Responsibilities

• Perform psychosocial assessments of patients and families to identify needs.
• Develop and implement a bereavement plan of care.
• Educate patients and families to overcome barriers and align care with learning abilities.
• Document assessments and interventions in the EMR.
• Coordinate with IDT to develop and adjust the social work plan of care.
• Identify and address end-of-life and advanced directives needs.
