Qualifications

• Ability to work as part of a team.

Responsibilities

• Provide psychosocial assessments of patients and families to identify emotional, social, and environmental needs.
• Develop a bereavement plan of care for family/caregiver needs.
• Educate patients and families respecting faith and culture traditions.
• Document psychosocial assessments and interventions in the EMR.
• Implement social work plans collaborating with the IDT.
• Support patient decisions and care planning.
